I acknowledge that after my free staff consultation, Shirley’s Initial Consultation for Integrative Mental Health Services is $500, her Dynamic Duo Hypnosis is $3,000 and her Quit Smoking/Vaping Dynamic Duo Hypnosis is $3,500.

I understand that Shirley does not bill insurance directly and her fees are my responsibility. If needed, I can get a superbill to submit to my PPO insurance for reimbursement. *PPOs typically reimburse 50% or more of Shirley's fees if the annual deductible has been met.

I understand that Shirley is a Non-Provider for Medi-Cal and Medicare. Shirley does NOT bill Medicare.
